The following pages give details of North Wales Aesthetics Clinic terms and conditions of service. If you require or need any clarification, please contact us and a member of staff will be happy to help you. Please be aware that any information provided through our website or brochures does not constitute professional advice. No professional advice will be given without a direct one to one consultation with your consultant Karen Johnson or other.

North Wales Aesthetics Clinic is a private clinic and does not have a contract with the NHS and all treatments provided are on a private basis only. Please ensure that you have read and fully understood our terms and conditions prior to booking an appointment.

Special requirements

Please let us know if you have any special requirements such as a disability, hearing or sight difficulties or the need of an interpreter. North Wales Aesthetics Clinic is able to provide access to the cinic, toilet facilities and treatment rooms for clients with wheelchair mobility needs.

You must inform stafff if you have any medication or food allergies and intolerances.

Please inform us in advance if you are bringing someone with you who is under the age of 18 years so that we can make any necessary provisions.

Are you considering having treatments?

All initial consultations are free and allow you to have uninterrupted time with your consultant who will be responsible for your treatment and care if you become a client of North Wales Aesthetics Clinic. During your consultation you will be shown before and after photographic evidence of your consultants work to allow you to see what you can achieve and the likely outcome of treatments performed at North Wales Aesthetics Clinic. The treatments, procedure, side effects, any risks and potential outcome of having treatment, will be discussed in detail with you. You will have the opportunity to ask questions and have comprehensive answers. Following a detailed thorough consultation with your consultant, a treatment plan will be set by yourself and your consultant. Details of your treatment plan, pre and post treatment advice and costs will be given to you in writing. Some treatments can be performed on the day of your consultation if appropriate and if you are assessed as suitable.

What will happen when you attend the clinic?

When you arrive at the clinic you will be shown into the waiting room. Please feel free to browse leaflets and photography and help yourself to refreshments.

Karen or your consultant will introduce themselves to you and you will receive a full comprehensive, private and unhurried consultation about the treatment that interest you or are suitable for you. We encourage clients and your relative or friend to get involved with your care at every point. Your treatment, procedure, any side effects, risks or complications and post treatment care will be discussed and a plan of care will be prepared with your full co-operation. Please feel free to ask questions and raise any concerns that you may have. We aim to alleviate all your concerns before you commit to having treatment.

Some treatments can be performed on the same day as your consultation but only when you have fully understood about your treatment and your consultant feels that you are suitable.

Who will perform your consultation and treatments?

Procedures are performed in a spacious well equipped, private and relaxed clinic by Karen Johnson an advanced trained consultant. Professor Paul Edwards performs a range of up to date vascular and varicose vein treatments.These consultants are supported by a team of assistants who are suitably experienced and professional. Assisting staff will be on hand to assist with procedures and to help and support you.

New clients medical assessment and consent

New clients are required to complete a written medical assessment containing questions relating to your past and current medical and medication history. This will allow us to assess your suitability for treatment. And you will have a series of treatment photography before any treatment is carried out. Photographs will be sent to you so that you can assess and monitor the outcome of your treatment. You will undergo a thorough consultation prior to having treatment you will have the opportunity to discuss the treatment, your goals, expectations and ask any questions. During your consultation you will be informed comprehensively, if there are any risks associated with your treatment. You will be required to complete a consent form prior to having any treatment. North Wales Aesthetics Clinic complies with the data protection legislation. All information given to us by you, will be held in the strictest of confidence and under no circumstances what so ever, be passed on to any third party. Clinical records will be securely stored for a minimum of 9 years prior to final permanent disposal.

Treatment suitability

Prior to having treatment at North Wales Aesthetics Clinic you will be assess whether treatment is suitable and appropriate for you and likely to be successful. If you are not suitable for treatments we will inform you of the reason why and you will be advised of a more suitable treatment or alternative. North Wales Aesthetics Clinic can not guarantee that you will achieve the same results as North Wales Aesthetics photograph gallery or promotional material such as brochures. We can not guarantee the lasting effects of treatments, however we will endeavour to give you an accurate treatment outcome whenever possible.

Treatment after care

Following your procedure you will be given comprehensive advice and a post treatment leaflet with any special instructions, requirements and after care. You may require a follow up appointment and this will be made for you prior to you leaving the clinic. In some cases you will be given a date when you will be required to call the clinic to make a follow up appointment.

Medical photography

Photographs will be taken before and in some cases after having treatments so that you can monitor the effects of your treatment. Your photographs will be kept in your notes and viewed only by your consultant and clinic staff who administer your treatment or who are involved with your care. You will have to give permission to allow the clinic to use your photographs in the clinic portfolio.

Aesthetic correction service

Karen is an advanced trained aesthetic consultant and she is trained to perform correction of Botox® and dermal filler treatments. If you have had a treatment performed elsewhere and you are not happy with the balance and symmetry of your face then we will help you.

Chaperone service

You have the right to be accompanied by a chaperone during any examination or procedure. One of our clinic assistants will be assigned to you and stay with you at all times during your visit. Please feel free to befriend your assistant and ask her questions or advice. You can bring a family member or friend with you who can stay with you at all times, confidentiality permitting.

Limited liability

No client will receive any form of treatment at North Wales Aesthetics Clinic until the client has filled in an accurate and truthful medical assessment form, had appropriate photography and signed a consent form.This will be required after undergoing a full comprehensive consultation. Such details will form part of the clinic terms and conditions.The terms and conditions along with the consultation, medical assessment and consent forms represent the full agreement between North Wales Aesthetics Clinic and each client and no form can be amended with out permission of North Wales Aesthetics Clinic. It is every client’s responsibility to ensure that North Wales Aesthetics Clinic has been provided with all relevant medical details prior to having treatment. North Wales Aesthetics Clinic will not be liable for any damage that occurs as a result of the client’s failure to disclose such details. The client hereby undertakes to comply with all instructions and recommendations given to them by North Wales Aesthetics Clinic regarding the aftercare of any treatment completed. North Wales Aesthetics Clinic accepts no liability of any kind incurred as a result of the client’s failure to follow such instructions and recommendations. In no event shall North Wales Aesthetics Clinic be liable to the client for any loss, indirect or consequential loss or damage whatsoever.

Appointments

You are welcome to arrange a free consultation for advice only. To avoid disappointment please book your treatment appointment in advance. Appointments can be made by calling the North Wales Aesthetics Office on: 0800 028 4744. All messages left on our answer service will be returned within 48 hours. Under no circumstances will we discuss the reason for your call with any one else who may answer your phone or disclose who we are. North Wales Aesthetics Clinic staff do not leave messages on landline telephones. Please arrive at least 10 minutes prior to your appointment time to allow time to check in, relax and prepare for your treatment. We will endeavour to ensure that your appointment runs to time. On occasions for reasons beyond our control, the clinic appointment system can run late. By booking an appointment you are agreeing to co-operate with North Wales Aesthetics Clinic in full in terms of our conditions as outlined.

Late for appointments

Our appointment system is designed to allocate the correct time for your treatment. Unfortunately if you are late this may result in your treatment time being reduced or you may be required to reschedule your appointment. We understand that some clients travel long distances to get to the clinic, and in some cases being late for appointments can be unavoidable. We make every effort to see clients on time, however for no faul of North Wales Aesthetics Clinic, on occasions the clinic does not run to time. We do not wish to cause any inconvenience to our clients and in advance we apologise for this. 

Cancellation of appointments

You will be notified by telephone, email or letter of the date and time of your appointment. Your appointment will be booked and reserved for you. It is your sole responsibility to attend your arranged appointment. If you are unable to attend for your appointment please give 48 hours notice in advance so that your appointment can be given to someone else. Should you fail to attend the appointment without the appropriate notice, prior to being offered another appointment, you will be required to make a payment of 50% of your treatment fee. Should you cancel 3 appointments you will not be offered another appointment. After failing to attend for a free consultation or having to cancel your consultation, we will require a £50 deposit prior to being offered another appointment.The £50 will be deducted from treatment cost if you wish to proceed to having treatment. Deposits are non refundable if you do not attend.

In very rare occasions that we may need to cancel or postpone your appointment we will make every effort to cancel your appointment in advance. We do not offer compensation if we cancel appointments.

Cancellation of a course of treatments

Coolsculpting and varicose vein treatments must be paid in full on the day of your first treatment. If you are unable to complete a course of treatments, after discussion with the clinic management we may be able to agree a part discount or complimentary treatment as a satisfactory settlement.

Making payments

North Wales Aesthetics Clinic accept payment by card or cash, payment by cheques are not accepted. A surcharge of 1.5% will be added to credit card payments. You will automatically be informed of this by a member of staff should you present us with a credit card when you are making your payment. Cost of treatments and products can be found on North Wales Aesthetics Clinic brochures. We operate the right to change prices from time to time and you will be informed about any price changes prior to having your treatment. Payments are made at the time of having treatment or making purchases.

Clients from outside the UK must make payment for treatmentsor products in GB sterling. All prices are exempt from VAT.

Gift vouchers

Gift vouchers can be purchased towards any treatment or product. You can purchase a gift voucher for any value by calling the office on: 0800 028 4744. Vouchers are non-refundable and can not be exchanged for cash. Vouchers for specific clinic promotion or special offers can not be used towards another treatment. If you use a gift voucher towards a treatment or product that is lower than the value of the gift voucher, no change will be given but you can obtain a credit for the remainder towards another treatment or product. All gift vouchers are valid for 6 months and treatment must be completed before the expiry date. Please allow 3 to 5 working days for processing an order for a gift voucher.

Orders for delivery

All orders placed for delivery must be paid for prior to dispatch and a postal charge will be added. You will be informed of the full amount prior to dispatch. Parcels will be sent to the address supplied by the customer.

Products purchased from North Wales Aesthetics Clinic are not to be resold or used by anyone else unless discussed with North Wales Aesthetics Clinic.

Personal details

It is very important that you provide full medical history, details of any medication that you are taking and any pregnancy status. Should these change in any way it is very important that you inform a member of staff. It is every clients responsibility to inform the clinic of any changes in either your personal details or medical history.

Please inform us of any change of address so that your records and our data base can be amended.

Use of client personal details

At North Wales Aesthetics Clinic we like to keep our clients informed of important clinic changes and of our latest special offers in the form of a newsletter. We also like to remind our clients of appointments, when you may be due for another appointment and various other important reminders. On this note you may be contacted by telephone, email or by letter in the post. If you do not wish to be contacted by the clinic by any or all of these means, please let us know. It is important to let us know either by telephone call to the office or in writing by letter or email of your change of address.

Confidentiality

North Wales Aesthetics Clinic take confidentiality of our clients very seriously and all employees of North Wales Aesthetics Clinic strictly adhere to Data Protection Principles contained in the Data Protection Act 1998. We respect the confidentiality of your personal information, records and privacy.

Data protection act

North Wales Aesthetics Clinic store all clients personal, photography, medical and treatment information on a secure computer system in accordance with the data protection act and the GDPR. All clients clinical notes remain the property of North Wales Aesthetics Clinic. Copies of notes can be made available on written request and North Wales Aesthetics Clinic has the right to charge an administration fee for these.

Infection control

The control of infection is a priority at North Wales Aesthetics Clinic and the incidence of infection is to date extremely low. We take great care to reduce the risk of any client contacting an infection by putting into place strict and effective training and discipline of all staff in accordance with the department of health best practice recommendations.

Hand hygiene is the most effective way of preventing and controlling infections and you can help us by washing your hands after visiting the toilet facilities and using the hand gel sanitisers which can be found around the clinic. For your reassurance we would like you to observe how staff prepare trolleys and equipment for your procedure and how often throughout your procedure that we adhere to our hygiene rules and polices.

Pregnancy and breast feeding

Some of the treatments that we offer are unsuitable for anyone who is trying to conceive, is pregnant or breastfeeding. It is imperative that you inform a member of staff of your status prior to having treatments.

Queries and concerns

North Wales Aesthetics Clinic website, literature, leaflets and brochures have our contact details should you need advice out of clinic and office hours. Leave a clear message on the answer phone and a member of staff will return your call as soon as possible.The office phone is accessed on a 4 hourly basis. If you feel you need to be seen by a doctor in an emergency, please contact your General Practitioner or go to your nearest Accident and Emergency department.

Client questionnaire

We would be grateful if you would complete the client questionnaire provided and you can post it completely anonymously in the questionnaire box at reception. The feedback that you provide about our staff and services will be useful in helping us to maintain and improve the quality of the service that we provide for you. All comments and suggestions are taken seriously and are vital to our drive in improving our service.

If you are happy with your experience at North Wales Aesthetics Clinic then we would like to hear from you. Should you feel that you are dissatisfied with any of the care or treatment that you have received there is a leaflet in the clinic entitled “a guide to making a complaint.” All complaints or concerns no matter how small are taken very seriously and will be dealt with quickly and effectively so that we can get it right.

Your rights to make a complaint

North Wales Aesthetics Clinic endeavours to treat all clients appropriately with fairness, dignity and compassion. We always take complaints about any aspect of our service very seriously, in order to ensure that every client has only the very best experience at all times. However if you have an issue with the way that you have been treated, a member of staff, your treatment outcome or any other matter relating to your visit or treatment at North Wales Aesthetics Clinic, you are entitled to make a complaint. You can make a complaint either verbally to a member of staff, by telephone call or in writing by post or email. North Wales Aesthetics Clinic will be pleased to help and advise you if you require assistance with this.

North Wales Aesthetics Clinic has a complaints procedure in place and we make every effort to assist you to a speedy and satisfactory outcome. Once we have received your complaint you will receive a letter within 14 days and you will be informed of the way that we intend to investigate the matter for you. During the investigation you may be required to attend the clinic for a meeting with the staff involved with your complaint. You can choose to see any member of North Wales Aesthetics Clinic staff who you feel comfortable to communicate with. We will strive to achieve to resolve the situation for you as quickly as possible.

No tolerance of abuse policy

At North Wales Aesthetics Clinic we operate a zero tolerance policy to abuse to our staff and property in any of the following behaviours: persistent missing appointments and late cancellation of appointments, acting in a demanding nature, being disrespectful to staff or shouting and arguing. In these or other situations, North Wales Aesthetics Clinic reserves the right to refuse treatment and admission and you will be removed from the clinic client base.

Smoking policy

North Wales Aesthetics Clinic is a strictly no smoking premises. There is a designated smoking shelter in the grounds of clinic premises located in the garden area.

Disabled visitors

North Wales Aesthetics Clinic promotes accessibility for disabled visitors and the clinic has the benefit from a lift, wheelchair accessible doors throughout and disabled toilet facilities. All treatment room beds operate on a low level for easy transfer from a wheelchair.

Mother and baby facilities

North Wales Aesthetics Clinic has the benefit of baby changing facilities. Whenever possible it is advisable not to bring babies and small children to the clinic unless you have an accompanying responsible adult who can supervise a minor. It is impractical to observe children at all times and the clinic environment is unsuitable and may be dangerous for children.

North Wales Aesthetics Clinic will not be held accountable or accept liability of any kind for any liability incurred as a result of children on the premises. In no event shall North Wales Aesthetics Clinic be liable to the client for any loss or for any other indirect or consequential loss or damage whatsoever.
